Tattoo Release Waiver

The following is a waiver, release and consent to tattoo and must be completed before the commencement of any new tattoo at FLT Tattoo Studio. This form is in compliance with NSW tattoo licensing laws and insurance requirements for tattoo studios in NSW. You must be 18 years or older and be of sound mind to complete this form and get tattooed at FLT Tattoo Studio.
